How to Login In To UTSW MyChart Account
Step 1– In order to access your account online, you must visit the login homepage. Once the web page opens you will find the login section at the top right.
Step 2– Provide your MyChart Username and Password in the blank spaces. Finally, click “Sign in” and you are ready to go.
Forgot My Chart Username
Step 1– If you have lost your MyChart Username go to the login homepage then click on the link that reads “Forgot MyChart Username?” (For direct access to the username recovery page, click here Link).
Step 2– In the next step, you will be required to verify your personal information. If you have an email address on file a new username will be sent via email.
Step 1– Lost your MyChart Password? Click on the “Forgot Password?” link found at the bottom of the space to enter the password, as shown below. (Use this Link to directly approach the password reset page).
Step 2– Provide the following information to reset your MyChart Password:
- MyChart Username
- Date of Birth
- Zip Code

Step 1– To sign up for a new account you need to visit the sign-up page of MyChart.
Step 2– Once the web page opens, provide the following information to identify yourself:
- MyChart Activation Code
- Date of Birth
